Capture when a Task is either Performed/Refused

The mobile app needs to allow the DCS to report whether each task is Performed/Refused/Skipped and provide reporting on what tasks for each visit are performed/refused/skipped

    1. Is skipped required as an alternative to Performed/Refused?

      1. Could use the refused reason drop down to provide the context for what the task wasn't performed.

    2. Do we force the DCS to make a selection of what performed/refused no matter what?

    3. Will need the drop down reasons for why items were refused
