Care Coordination App: View DCS Secondary and Tertiary Phone Numbers

Currently unable to see Phone 2 and Phone 3 from the Care Coordination App when viewing the DCS Profile (DCS List).

Unable to contact DCS on any other phone numbers besides Phone 1 when out in the field.
