POC Care Coordination App: Visible Schedules from DCS tabs with client name and phone number

Improved Visibility of Client–Caregiver Relationships

Current Challenge:

Under the DCS tab, client names and phone numbers are not visible.
Under the Client tab, caregiver names and contact details are missing.
As a result, CSMs must manually contact caregivers to determine which clients they’re assigned to, especially when managing call-outs or coverage.

Idea Benefit Improves responsiveness during urgent scheduling changes. Makes the app more intuitive and functional for day-to-day operations, more ideally for weekend coverage when a CSM doesn't have their laptop or while in the field. Attachments Open full size
