Enable ability to enter individual notes per service for concurrent services

During concurrent services, there is one note that is populated across all service entries instead of allowing distinct documentation for each service.

Idea Background

The ability to maintain separate notes is critical for us in Alaska. If respite-related tasks appear within PCS service documentation, it can result in payback during an audit. Auditors are extremely particular about task selection and ensuring that notes align precisely with the authorized service delivery records.
