Add a standardized dropdown field in the EVV “Client Signature” section to identify who is signing the visit attestation.

Add a dropdown list in the EVV “Client Signature” section that allows the user to select who is signing the visit verification.

Suggested options (Considering all LOB):

  • Client (Self)

  • Legal Guardian

  • Power of Attorney (POA/HCPOA)

  • Parent

  • Spouse

  • Adult Child

  • Other Family Member

  • Designated Representative

  • Case Manager

  • Residential Staff

  • Unable to Sign – Physical Limitation

  • Unable to Sign – Verbal Confirmation

  • Refused to Sign

  • Not Available at Time of Visit

  • Caregiver Attestation Only

If a selection other than “Client (Self)” is chosen, require entry of the signer’s name, relationship to the client, and reason the client did not sign.

Idea Background

Currently, the EVV system captures a client signature but does not identify who actually signed when the client is unable to do so. This results in inconsistent documentation, reliance on free text, and limited ability to validate that the signer was authorized.

This gap creates risk during audits or payor review, as there is no structured way to confirm whether the signature represents the client, a legal representative, or another individual. Caregivers and branch staff are affected due to unclear expectations, and auditors/case managers lack visibility into who verified service delivery.
