The PS Assessments module does not currently provide an option to remove, edit, or replace an assessment that was entered or uploaded in error. If an incorrect assessment is selected, an incorrect effective date is entered, or the wrong file is uploaded, the user is unable to correct the mistake within the system. This can result in inaccurate records and additional time spent finding workarounds or requesting assistance to resolve the issue.

Adding the ability to delete, edit, or replace an assessment entered in error would improve data accuracy and user efficiency. Users would be able to correct mistakes immediately rather than maintaining incorrect records or seeking system support. This enhancement would reduce administrative burden, improve record integrity, and provide greater confidence that assessments accurately reflect the individual's information. While this issue does not prevent assessments from being completed, incorrect assessment records or effective dates may lead to inaccurate documentation, reporting errors, and potential audit concerns if errors cannot be corrected. Providing a controlled method to edit or remove erroneous entries would help maintain accurate records and support compliance requirements.
