Adding Electronic Errand Form to EVV for Aides to complete if errand mileage is reported

This is the only paper form that the DCS has to turn in if they are completely electronic documentation. This would eliminate this form and remove the human error if mileage is reporting by having it electronic. Client and Aide both sign, see example of paper form below.
